d11f466b1a The integration test had highlighted that the load control type was not being hidden correctly. Found that it was referenced as load control point, fixed to load control type.

2054eb96d6 Fix for #368 any details entered on the enquiry that were not explicitly saved were lost when the user attempted to upload a document. Altered the upload button to ensure that all changes are saved back to the database before the file is uploaded.

d6995856a2 Addresses #374 by ensuring that the stored rejection reason is less than 2000 characters. 
NB. APEX Text Box with Counter does not include carriage returns in its count and will return a string longer than the maximum defined.

24c31ebc45 Add calc_meter_reqs to mip_quotation.pck. 
Ensure that all calls to add_quote_reason use correct parameters (mip_quotation.pck).

Change grant from 'CREATE ANY VIEW' to 'CREATE VIEW' in InstallMIP.sql following problems installing 'supporting objects' into MIP_TEST.

a40b896dd5 fix for ticket #365, the code was trying to get the supplier address changed this to the agent address, also the code was never going to get an address as the call to get the address occurred before we got the supplier/agent ids, so it was always passing a null as the supplier/agent id.

63a5951bd1 for ticket #354, set the apau.ctl control file to accurately reflect the page security model. Removed the file from the different builds and updated the seed version of the file. Now we'll have one consistent file for all builds.
