New package mip_debug_constants.pks to be used to store compilation constants for debugging purposes (packages to use pl from mip_debug).
mip_quotation.pck modified to support exchanges based on being provided with an existing model rather than meter size. Changes to the DATAITEM_ROLES.csv to support this new field, impact on mandatory rules captured by mip_helper_special_cases.pck

Add processing to make it clearer which application build has been produced and to link the schema with the correct application (AssociateSchema.sql, SetSubstitution.sql, bulk_load.sql and InstallMIP.cmd)
Changes to schema: METERS now have 'MESC_CODE'. Associated changes in the bulk_load routines and initial changes to mip_quotation.pck (needs confirmation testing).

Add constraint to regi_enqu_exclusions to prevent both enquiry and additional items being recorded on the same row.
